Step-by-Step Referral Instructions:

  1. Download the Referral Form: Please start by downloading our editable PDF referral form here. Complete all possible sections to ensure a smooth transition of care.

  2. Gather Patient Records: Along with the completed referral form, please include:

    • Recent progress notes

    • Relevant lab results

    • Copy of the patient's insurance card

  3. Fax the Documents: Send all documents to Rixis Dermatology via fax at 614-887-7723.

Insurance Information:

Rixis Dermatology accepts all major commercial insurers and Medicare. Please note, we do not accept Medicaid plans at this time.
